Lakewood Ticket to Play

Hello LMMS Family! It is that time to prepare to kick-off athletics again, here at Lakewood. And with that comes the wonderful ticket to play process.

To be able to tryout for a sport at LMMS, all students must obtain a ticket-to-play (TTP). This ensures coaches that students have brought me all required forms, meet academic and attendance requirements (7^th & 8^th graders only for the fall season), and have registered online. Tickets will be wristbands that will be passed out during all 3 lunches on the first day of school.

TTP Sign up days: 

August 15^th – (Bridge/6^th Grade Only) : Information Session @ 10:30a

August 19^th – 7^th/8^th Grade Only : 10am-12p

August 21^st – All grades (during open house)

Wristband (TTP) pickup: 

During all 3 lunches on the first day of school

Prep: 

Monday August 26th 

(Times TBD)

Tryouts: 

August 27th-28th 

NO TTP will be given out after 11/13/23.

These are the following things needed to obtain your ticket to play:

1. A HARD COPY of your completed NCHSAA physical
2. A HARD COPY of the Gfeller-Waller concussion form
3. A HARD COPY of the HIPPA form (found on DragonFly)
4. A completed online registration through DragonFly 

**ALL OF THESE FORMS MUST ALSO BE UPLOADED TO DRAGONFLY**

**Please remember that all students in the 7th & 8th grades must be in good academic standing and not have more than 13 absences. **
